By submitting this form, I acknowledge and agree to the following:

Purpose of this form:
This application is to express interest in participating as an artist, assistant, or volunteer in the Imperial Avenue Arte West Project and related mural programs coordinated by Barrio Artists Partnership (BAP).

Non-binding application:
Submitting this form does not guarantee selection for a mural project or paid position. Participation opportunities depend on project needs, funding, and community partnerships.

Use of submitted materials:
I affirm that the artwork and images I submit are my own or that I have permission to share them.
I grant BAP and its partners permission to review, reference, and display my submitted materials for non-commercial use, including project planning, selection, promotion, and community engagement.

Media and publicity consent:
I consent to the non-commercial use of my name, artwork, and likeness (photos or videos) in connection with the Imperial Avenue Arte West Project and other community art initiatives.
This includes social media, press releases, and documentation for public reporting or educational use.

Contact and communication:
I authorize Barrio Artists Partnership to contact me regarding this and future art opportunities via the methods I have indicated (email, phone, text, etc.).

Data privacy:
All personal information submitted will be used solely for the purpose of artist coordination, project updates, and program evaluation. It will not be shared or sold to third parties.

Voluntary participation:
If I choose to volunteer, I understand I am offering my time and skills voluntarily, without expectation of compensation, unless otherwise agreed upon in writing.
